What Is Benzoyl Peroxide?

Benzoyl peroxide (BPO) is an organic compound and one of the most clinically validated over-the-counter acne treatments. It works by releasing free oxygen radicals into the follicle, creating an environment toxic to Cutibacterium acnes (formerly P. acnes) — the anaerobic bacterium central to inflammatory acne. Unlike antibiotics, BPO does not cause bacterial resistance, making it a durable long-term option.

How It Works

BPO penetrates the follicular opening and decomposes into benzoic acid and oxygen free radicals. The oxygen kills anaerobic bacteria while the mild keratolytic action helps loosen comedones. It also has anti-inflammatory properties by reducing the bacterial load that triggers the innate immune response cascade causing redness and swelling.

Concentrations

Available from 2.5% to 10%. Studies consistently show that 2.5% is nearly as effective as 10% with significantly less irritation, dryness, and bleaching. Starting at 2.5% and increasing only if needed is standard dermatological advice. Leave-on treatments are more effective than wash-off formulations.

Key Precautions

BPO bleaches fabric, hair, and towels on contact — use white towels and pillowcases. It should not be layered with retinoids in the same routine — use BPO AM, retinoid PM. It is a strong oxidiser and will degrade Vitamin C serums if applied simultaneously.

Resistance note: Unlike topical antibiotics (clindamycin, erythromycin), benzoyl peroxide does not induce antibiotic resistance — a major clinical advantage for long-term acne management.
